2019 Application Form – North America:

Please note this application form is for the CHEK Academy, Year 1 commencing October 1, 2019 in North America.

Course dates will be maintained on our website and we advise that you view these prior to making your application. It is essential that you are able to attend all dates and will be required to confirm this during your telephone interview.

The fees for joining the CHEK Academy will depend on which point you are applying to enter if you are unsure please call +1.760.477.2620 and ask to speak to the CHEK Academy Coordinator who will be able to assist you in this matter.

If you have purchased any of the items included in the academic plan after the point which you are applying to join, these will be deducted from your monthly fees. Please provide details on application form to your revised fee to be calculated accurately.

PLEASE NOTE: The price and payment plan below includes Years 1, 2 and 3. The payment of $394 over 36 months covers all 3 academic years.

Year 1 Fees
Monthly Fee is $394 a month for 36 months. The first payment is due to reserve your place on the academy within 7 days of being accepted. Followed by 35 monthly payments of $394 commencing on the October 1, 2019. Payable by Credit/Debit card subscription.

The CHEK Academy is only open to student who are aged 18 years and over please confirm you are 18 or over.

While there are no specific pre-requisite qualifications required prior to joining the CHEK Academy, please provide any current qualification s or memberships to professional bodies below.

C. Previous Experience

Please provide details of previous experience you have working with people as a Health and Fitness Professional that you believe is relevant to the training you will receive during your time in the CHEK Academy. If you have no relevant experience, please put 'N/A' in this field.

D. Preparing for Learning

For your time with the CHEK Academy to be successful, you need to maintain a good level of motivation throughout your studies. The key to completing any course of learning is to keep the enjoyment factor high throughout the entire process.
